Become a Hudson Crest Patient
Hudson Crest Health welcomes new patients across all five boroughs. Existing patients can log in to MyChart; new patients can book online or by phone without an account.
1. Request an appointment
Book online or call us. New patients do not need an existing account to schedule.
2. Complete your intake form
Download the new patient intake form below, fill it out, and bring it with you.
3. Arrive 15 minutes early
On the day of your visit, arrive 15 minutes before your appointment time for check-in.
How to book
You can book a new patient appointment online through our appointment request page, or call our new patient line. You do not need a MyChart account to book your first visit.
MyChart for existing patients
Existing patients can log in to Epic MyChart to view records, message your care team, and manage appointments. New patients can book without an account, and you will receive a MyChart invitation after your first visit.
What to bring
Please bring the following to your first appointment:
- A valid photo ID (driver's license, passport, or state ID)
- Your insurance card
- A list of your current medications, including doses
- Your completed new patient intake form
- Any referral or prior medical records, if you have them
